The context of the situation:
Ines was pregnant with their third and last child. She and Carcel were separated by her choice and they had sex one night and she got pregnant even though he used contraceptives. Since they were separated, he didn’t know she was pregnant and Ines made sure to hide her pregnancy from him and her family by going to a different part of the country to live. She was already seriously ill and she knew the risk of giving birth. She was willing to die to give birth to this last child and she hoped that child will live on in the world as proof of her and Carcel’s love.

Unfortunately, she returned to Mendoza for an event and fainted while at the event. Carcel was there and took her home to the Valeztenas where she was staying since they were separated. There the doctor attended her and they found out she was pregnant. They also got the difficult news that it’s highly likely she’ll die giving birth. Ines’s father wanted to make the decision to abort the child but Carcel said he would make the decision instead and take that sin upon himself.

Why was her family and Carcel not willing to risk Ines’s life? Because the other two children that Ines gave birth to had died. They were poisoned to death but Ines and Carcel didn’t know it at this point of the story. Anyway, both deaths were very painful for Ines and Carcel and one can say the deaths of their children is why Ines demanded a divorce from Carcel since he was the heir and needed an heir. Ines was also deeply mentally abused by her mother in the first timeline so her psychology was complicated and it’s compounded by her stubbornness. Carcel begged her not to divorce him since he can make his younger brother his heir. Ines refused this option and wanted him to find a wife that can give him children and a better life. Carcel, of course, refused and stayed faithful to the end.

Anyway, when Ines woke up, Carcel told her the child was gone. That’s when she said those hurtful things to Carcel that we see at the beginning of this ep. Despite all of this, the reader knows they truly loved each other. At the time, all of Mendoza and maybe even Carcel thought only Carcel loved Ines and his love was unrequited. That’s why even in this current timeline (the 4th TL), the crown prince, who has his first TL memories, never believed that Ines would fall for Carcel because all of the nobles in Mendoza in the first timeline thought Ines didn’t love Carcel.

After Ines tells Carcel never to appear before her again, they separate for a long time. He went back to Calztela and went to many battles. She went to a far part of the country to die. However, we know they reunited because we already saw she died in the little house in Calztela.
