The female antagonist's fake pregnancy actually is based on a real medical condition.
It's called pseudocyesis. The afflicted person feels pregnancy related sensations, including feeling a baby kick and experiencing morning sickness. It's very sad and I can't imagine what that would do to someone psychologically.
