Clearly not dementia. It's a progressive illness with no cure. I guess he had a brain tumor or something and went back to the US to undergo surgery. Seeing that he remembers everything just fine now.

Anything similar to the "third ending", where the uke fell first but the seme fell harder or something like the seme rejected the uke at first but then regrets it after.
