I think it’s because the bottom was alone during the time he got in the accident and if the top was there he could’ve helped with the bottoms recovery and supported his comeback.
Lee yeon cried because he thinks that him cutting off kihoon in the past was selfish to do...or else yeon would have been there to comfort kihoon during the accident period. That's why kihoon says that yeon does not have to feel guilty about him leaving soccer...hope this helps
