They mentioned the MC father was a spy (kinda of) so he had to be a high ranking one to infiltrate in the fathers business/home.
Another theory of mine ,which is a wild one, is that the twin's father mpreg the MC's father and then he gave birth to mc which could explain why the twin's father took care of MC this whole time.

It's the panel layout. The art is AMAZING imo but the panel layout makes it a little hard to understand
