But the story is about a girl who gets transported into an otome game (or novel) and builds good relationships with all the male leads, but then they all leave her.

There’s a lot of traumatizing things that Seth goes through. Not to spoil exactly, but the main antagonist of the manhwa rapes him and he becomes extremely traumatized, and it shows in many situations, and in some of those situations, he is being SA’d. It can get really heartbreaking at times.
In a few specific chapters there is a traumatizing very realistic depiction of brutal sexual abuse in all its horror and sorrow, the specific chapters to be careful with are: season 1's chapters 31, 32, 43, and the first half of chapter 44. In season 2, it's chapters 94 and 122
