For me it's always the "bestfriend" coming into the story at a random time when the MC are already in a committed relationship then it just leads to misunderstandings
The day Japan stops pairing minors with adults, or randomly throwing a loli into every other manga, will be the day the world heals a little. Because honestly, why are so many yuri manga plagued with this?
