I've dropped manga and manhwa solely because one of the love interest looks like a child. Whether it's bl or straight romance one of them looks like a malnutritioned small child.
There's also instances in a bl where the bottom suddenly looses all his body mass/muscles and is obviously smaller. I've found that this usually occurs over many chapters/seasons.
Ex) the nurse guy from jinx (i dropped it).

Oh the list is SO LONG. I'm disgusted and furious cause it happens so often and for no valid reason. sometimes i wonder if its an art consistency or uh deliberate? to name a few: cozy obsession, rainbow city, can't think straight, solo for two, scent and sensibility, nobara to pretenders, happy omegaverse etc. despite the story being good in some of these, that lowkey implied shotacon is so offputting.
and since we are talking about this lemme talk about the better works on the spectrum: The Pawn's Revenge, Boku no high spec kareshisama, Doutei Tora to Tsundere Usagi, etc. which despite having twinks as their protagonists, ensure that there is a distinct mature adultness to them which is nothing childlike and I'm so relieved for that. As an adult reader, it makes more sense and turns the story into a better experience.
