If it’s about the usual type of gifts, you could think about a nice wallet or some kind of accessory. A trendy leather bracelet or a pair of sunglasses, for example.
Otherwise, depending on how appropriate it feels for the specific people in mind, a bottle of wine or whisky can make a good present.

As far as I know—though I’m in no way a professional—pain in dreams is considered rare and an indication of poor mental health.
How “real” the pain feels more so depends on the person, but not much is known due to how difficult dreams are to study.
