I went to a bunch of doctors cause I kept fainting and struggling to breathe. After being passed from one doctor to another, a cardiologist decided that my heart just happens to be fast and said I'm "like a chihuahua." Since then I've had an unhealthy obsession with ugly chihuahuas.

In these situations i just ask. Like i'd just say "Are we gonna kiss?" and depending on their reaction I'd act as if it were a joke or give them the smooch. These jokes are pretty common for me tho so it might just make things more awakward if youre usually more serious.
