Dietrich is the love interest and as far as the novel goes, they are endgame. . . Tbh, I was uncomfortable as well but this is different from Beware of the Brothers where there were parental figures raising them as siblings and they really act as a family. For this story, there was no one raising them and they really have to make do with themselves, from finding food to keeping themselves alive from assassination attempts. It's like a mini hunger games in their own residence and they depend on each other, so I view their dynamics differently. Kinda similar to childhood friends thing. Anissa definitely knows Dietrich isn't his brother so she never viewed her a sibling from the very beginning, I just don't know about Dietrich lol. . . Also in Asian culture, (specifically Korean) calling an older male "brother" or "oppa" is normal. Even in Filipino culture, we call slightly older men "kuya" even though we're not related. It's just a respect thing, but it really does become sketchy when translated to English lol esp because things are different in Western culture.
