I think in real life, Tokyo gets 3 representatives, so, from the other match between Nohebi (the snake guy) and Itachiyama, the loser will go against Nekoma to decide the third representative, and then go with Fukurodani and the other winner to the nationals.
I really hope this happens, because Nekoma has to go too!!! The third years need to be in the Battle of the Trash Heap! ╥﹏╥

I think that what you're talking about is not the representative play-off but the rankings. there are best 4,best 8,best 16 etc.
In rankings like the Best 4, the winner in the finals is will be the top school and their opponent is top 2.

Now,to decide who is top 3 and 4 in the Best 4, the teams who lost in semi-finals will have a play-off. In haikyuu right now,it's Nekoma vs the other team who will lose in the semi-finals (Itachiyama vs Nohebi)

Tho,I doubt Furudate-sensei will show it. :)
