I believe Damien is his dead brothers son. I recall the Duke being confronted by the Doctor “Old man” to pass down his bloodline and he admitted Damien was not his son. He is the heir to the throne only because the Duke does not want any children to pass down his bloodline. Vivian can change all of this by curing her infertility. The Duke and her have great chemistry and they finally confessed their feelings for each other. I believe this confession took place in the garden after they had a disagreement about her infertility issue. I am hopeful he’ll change his mind and want a baby with her.
