Yeah, that can be annoying - but please report them as missing when you realize. Usually mangago is kind of fast fixing such mistakes. There's even an extra field for missing chapters at the report flag, so there's no need to write anything besides the chapters number...
No I think it's more of uploaders fault. They aren't uploading some chapters/ might have missed it cause of numbering
