I think it was because of her disease (syphilis if I remember correctly)
it wasn't removed it deteriorated for the STD, she removed her own finger in to show him that she didn't want to brake her promise
People who had Syphilis often lost the bridges of their noses. It's a common result of the disease. They call it the "saddle nose deformity" which is a consequence of frequent infections attacking the cartilage.

So can someone clarify why she had her nose removed? Was that her punishment for getting pregnant?
