So in the final final chapter Ino and Uncle get married officially with all their children together and the triplets ...they are much older in the end (the triplets) and they all take a big family photo...it doesn't specifically say that Ino dies but it's implied that he will die of old age because he chooses to not to become immortal....the grave scene is just a representation that Elder will be there for Ino till his very last breath as well as when he's dead because he truly loves him and will still love him after he has died.
