I think it’s pretty realistic especially if they run in different social circles and universities/education/workplace. I’ve hardly seen the elder sister of one if my closest friends

It's possible, especially if (1) you're not actually that close (2) you just started being friends
Also, sometimes you just don't know some details about your friends
I think it's pretty realistic if you met when you're in college or older. At that point, most either move out of their family home or rather hang out away from their parents like a cafe. I don't know really know what my close friends' siblings look like. I've seen a picture or two in passing but I've never met them outside of maybe saying hi once or twice when they happen to be around.
I have a friend. I know her for a bit more than 10 years now. And she got to see the faces of my sisters only recently. They are much older, I'm not that close to them, we don't live together and I always meet this friend of mine outside of my family home. So yeah, it's possible.
