I feel like this changes depending on the story and how the author wants it to be. But usually they get pregnant when the Omega is in heat. The knotting usually increases the chance of pregnancy.
Iām pretty sure the omega can get pregnant without getting a knot. It depends on the story though, some people change it so the omega and alpha need the knot.
