When she was running away he said that “there is nothing that he don’t know so don’t lie to him” so I think he did know but did nothing but observed and manipulated her. And I think then biggest reason she hated him and ran away is that he just sit and watched while she is suffering w anxiety and pregnancy

If that was true he would have known that she had twins but he believed her when she said she didn't have a child.
Actually, the emperor offered help to elan behind the scenes. He does not want to force elan into telling him that they will be having a child. He wants elan to tell it herself. But elan keeps overthinking of her situation and came up with the idea that she will be killed if the emperor knew of her pregnancy. It’s basically just the two of them having miscommunication.
