In real life there's 4 type of transvestite
1. Got both reproduction organ but only man reproduction system be functional. Like can produce sperm.
2. Got both but only female reproduction system can be functional. Like hv womb and ovari.
3. Got both but both cannot functional well. This type can choose which one they want to live as but cannot produce baby.
4. Change of gender or reproduction system by surgery and medical works. Which they born normal but totally change it their gender.

So in this story kind love like type no. 4. Born normal but do surgery by putting vagina and change his hormones by drug/pill. But it make complicated and look like original trans when the brother do not remove his penis.. damn... Complicated story...
He wouldn't be a transvestite and that is an antiquated term. He wouldn't be transgendered either since he was assigned male at birth and (presumably) still identifies as male. I think the term you are looking for is "Hermaphrodite". As far as I know, IRL humans can't be hermaphrodidic like snails, but humans could still be intersex
