Yes, he did turn into a demon from eating Akagi's flesh. The last page, when Otsuji talked about his "living on love" lifestyle, it was a bit of a joke because it's a common phrase, but he also literally meant his eyes changed because he was "living on love" by eating Akagi.
Just to clarify, did Otsuji become a demon? His eyes changed and he was wearing sunglasse at night... And that thing he said about distancing himself form humans? Sounds like he's changing.
