Well, for me, it's more likely that Shin is aware BUT rejecting the idea of being Wolfgang's fiancee because he thinks is that he needs a QUEEN (who can give him an heir or something).

What pains me more than Shin neglecting his health is:
For Wolfgang, Shin is HIS fiancee and Shin is LOOKING for a queen for Wolfgang.
Correct me if I'm wring but the way I see and understand it, it feels like they are engage but only Wolfgang is the one who's aware of it.
Please just talk it out and set the politcs aside for a while
