The topic of incest was already covered some chapters back. They showed that the “John” we see is not her actual blood brother but a devil that came from her mother. The devil aka john only took on the appearance of a human after the mother made a wish/deal with a devil(?) that came from the coffins the mother’s family possessed. I think you can see this happen in chapters 32-33!
