It would be so hard to maintain and create a largescale online game like this ngl. Imagine the crashes and server lags. There's a lot of kingdom building/simulations out there but it doesn't have the relationship sim like aspect to it. It would be cool but kind of impossible cus you basically have to code personality and such for the vassals, dukes and citizens for each kingdoms.
Yeah thought so T_T but honestly I'd love any online game where you can choose your race/class and just roam around doing quests and talking to strangers, that would be fun.
I'm not sure if it's the latest installation but final fantasy 14 online might be good. I don't play it but my friend does, character custom looks good and there's multiple fantasy races that's not like dnd races(there's some similarities but not exact).
Unsure for their NPCs but you may meet a lot of people there. It's moreso an MMORPG

On another note, if i played this game I'd most likely be a goblin, living in a structural society underground, where everyone has a role and a job to fulfill.
