It's like, it became normalised to him due to the environment he lived in and he's become somewhat immune to it? By immune I mean his feelings towards it are turned off, he brushes it cuz he can't be weak or rather couldn't wait to heal and be weak. It was never a choice so I wonder if we'll ever have mokhwa sorting that out. It reminds me by violet evergarden when she was told that she's burning all over but she couldn't understand until later (If you watched it you'd get it haha)
