I don't see how you don't see that he doesn't and didn't have a choice. Nobody is else is or was ever going to offer him that type of money upfront. Maybe the 2nd supporting lead but let's be honest he gave up quick, and didn't see a problem with the way doc was being treated so I don't think he would've been much better. His only other option is go into deep debt again, and we both know where that would lead in this universe.

I understand what you mean. It's a really difficult situation indeed. And it's true what you're saying that the other options sucks. But that doesn't change the fact that he still had/has a choice.

We're conflating real life situations with the story. He got into debt because of the treatments, and it's MC's love for his grandma that keeps him going.

Of course everyone has a choice at every point in our lives, but narratively, it was explained well why in that specific situation, MC had their back against the wall and chose the lesser evil.
