Yeah that was very specific and oddly shaped shadows. They could have just not have him be naked. Or have the restraint belts running across his groin. It was confusing.
I think it is more dehumanizing. Which is the point. To the doctor and all the vampire hunters, "creatures" like Wren are not humans and don't deserve to be treated like one. So I think this was one way the author portrayed that. Not in a direct way, of course
I definetly think it was for the story telling purposes because some people might interpret that situation as kinky or something.(idk some of yall can be weird) It's supposed to be a dehumanising, violent and tragic situation., drawing a dong(iykwim) in it can lessen the impact or change the focus/feel of the scene if that makes sense? This is just my personal op but it also could've been a thing on the artwork side too. I do think like underwear would at least make a bit more sense when shadows but then again it is really dehumanising so it's probably what they're going for. ┐(´ヘ`)┌
