I'm surprised she didn't starve to death by this point

It's saddening. She's barely kept alive. Eat just enough for her not to be dead. Severe malnutrition is probably happened to her. Along with hunger she must feel depressed to. Hunger lead to depression too. Desperate to live yet desperate to die to. Can't even think how many years she's been clouded with suicidal thoughts.
