Readers are literally divided with opinions on who should end up with whom.
Personally I think whoever ends with up with whoever will be a sad ending, the plot is so twisted.

Like the Duke seems to like the doctor and the doctor definitely loves him.
Mac/mag is going through two emotional routes at the same time, one is being in love and the other is hating the fl which is totally possible. Even if fl tells him about her being a total different person I doubt anything will change since he is so stuck up on og reliana.
And ml looks like he loves the new personality of reliana but wants to continue believing it's the og reliana's real emotions kinda of a thing.
And it looks like he suffers from stockholm Syndrome and needs professional help.

Second to non the thing about the doctor deserving the queen position seems to be because she saved his life which is a possible reason and ml has no romantic feelings about it, it's likely like a duty as a emperor.

Ml can't live his position since their will be all kinds of uproar if that happens and everyone's life related to them will be in danger , the dark plains and the empire's relationship will continue to break if it's not someone from the plains rules the empire like ml and fl.

And if the plot goes like the original states, I think the last thing fl would do is run away to dark plains to return to her og world and ml regrets it since he has yet to sort his emotions towards og reliana.or maybe he follows her ( °з °)

I like the theory, one of the comments said about how fl might not need to die to return to the og world. About how og reliana seems to slip in from time to time and how fl is influenced so much by her emotions that she can't differentiate between her and reliana's emotions.

I think even in the original, ml kept reliana as his lover and tortured her mentally. And it seems it was already decide prior to fl's arrival ,of the future of cassios as the prince since their is no way that thought suddenly came in their head as if it wasn't planed like that in the original as stated.

I would say the answer to all this is the author and fl should quickly find a way to go their since her position in the empire is in shambles And before her weird obsession to ml increases.

The biggest obstacle is the temple and fl's blood line. If the temple is hold responsible for it's crimes people will say the emperor is really controlled by the devil since they don't know the truth. But if he resigns fl from her position everyone will force/pressure the ml to marry the doctor or the nobles daughter while he frantically searchs for fl and postpones any decisions about his empress position . I think this will be a good break for fl so she can travel to the dark plains and learn more and help the author for whatever she needs fl for but this would as well be as good as admitting the claims of the people and the nobles that the Duke (fl's father) family is made up of heretics so their is a great chance they will be demolished but unless, as the author has not told anything that great about what is the extent of the duke's power I think most likely he will somehow manage to keep his position and use this situation to bring his daughter back. I think this case is the most possible.
