Yes. I do agree. If it was then, consent between nobles and commoners never existed. It was good enough (probably) that hos majesty healed uke using an expensive gem. If it had been at the palace, the maids may eventually gossiped saying uke was treated favourably because that's what status gap meant back then. U just can't say no to royalties.
I'm reading this with an old mindset, and by that I mean if I was in their era I'd think that this was probably not uncommon. It doesn't make it okay but it does help me get through the story ╮( ̄▽ ̄)╭
