Yeah it's kind of sad that there are real world reasons for things that could explain a lot of wtf shit in manga but they never go there. It would really enrich the stories if they did. It would also help kids who read manga and see real world issues and are able to learn it's okay to be different. It's a not a stigma to have mental problems because everyone has something different they need to overcome.

Some Manhwas dig a bit deeper by having some of their semes kind of be psychotic or they make them a bit psychological but perhaps don't completely explain it.
