for the artist it is a taxing thing as you have two styles you have to switch between for the story, this tends to lead to rushed or underdeveloped stories as the artist gets a bit of burn out from it.

As long as it stays in the mangas and is not replicated irl, as some people seem to do, no it isn't bad for two consenting adults to have a relationship. i really was just mentioning the size difference because of the differences in art style and how it is trying to show a fetish. this manga has about 3 or 4 rolled into it.

people in real life can have things that big and are not able to have normal relations because of it. and death by screwing someone with a dick that large can happen, this is why women are always saying size doesn't matter.
The only time I have heard of death from doing it was when I was watching documentary about child brides and as I said I'm talking about normal relationships when there is big size difference but both parties are consenting adults and nobody is dying for it
