Well as far as I understood it, they meet again after a few years. Approximately after 8 years? They reconcile and they live together. At age 69 Akira dies from cancer(?) and Yuuki dies at the age of 82. Then there are a few pages how the two of them meet each other again in heaven. Akira waited the whole time for Yuuki.
Or was it 13 years? Well after a long time they reunited.

Even after seeing/reading the raws my heart still aches. It couldn't ease the pain which I felt in this chapter. Couldn't decide between happy or sad after that. I seriously cried. Geez.
