They are in reality, the game became a separate reality that humans got transmigrated into. The game creator/moderator needed enough "points" (in this case micro transactions) in order to turn her world (game) into an actual scenario, they haven't explained further than that, so I'm not sure if it's one of those saving my world situations or just creating worlds to create scenarios for people to "play" and accumulating power that way
So he played a game and woke after 5 years to find himself in the game world again. The synopsis seemed to imply that they would be in reality??? Can some one spoil?
