i haven't read it (and no longer plan to looking at the comments lol) but my guess is that it's more to do with how he's treated? like yeah it's not gonna feel like a bl if one of the characters is consistently portrayed as & treated like a girl, even if he's actually a guy.
it may not FEEL like a bl to some but that doesnt mean it isnt which is what a lot of comments i saw said

The whole thing is more on the comedy side, they know he is a guy and I never felt the were disrespectful with him or his condition, it wasn't soooo serious eother because the whole atmosphere never was, it never tried to explain or give a lesson just a fun ride of two idiots that fall in love and one can turn into a girl.

Most people have an issue because they "only" like yaoi, and didn't lke the "straight sex" element, and viceversa, which is super weird to me because if you come to a manga with gender-bender in the tag you are supposed to know what you are getting.
