I wonder if Leon is actually that other dude's (the god? Cliff's enemy?) "Good side" that he threw away in order not to be swayed from whatever his goal is. They're almost lookalike that when he first showed up in earlier part of the side story I really thought that was Leon.
Tbh, I've been trying to figure out what the whole 60+ chapters were trying to tell other than the sexual violence, but maybe author focused too much on that part they forgot to include the plot.. or they had to cut it out. I'm actually interested in the womb thing and why he couldn't conceive despite all the "methods" :( I'm only guessing but he probably couldn't get pregnant because of stress and/or some sort of mental rejection + body rejecting the whole pregnancy thing to survive the abuse.
Or maybe because Leon already gave his heart to another? Remember that story or lore in the early chaps ig where there were some race who changes their gender or whatnot for their beloved? Or am I mixing it with another manhwa?

Late reply, but you're right! I completely forgot about that part. Assuming he did that for Cliff and since his feelings are reciprocated/mutual with Cliff, maybe that's also why Nergal and everyone else couldn't get him pregnant?
