That's The point! Sihyun choose because in his eyes joohwan would tell him he loves him under any normal circumstances so if he said during sex he will get shocked and stop! And also because he obviously wants to hear joohwan say it even if it's a lie
Also sihyun seems so at ease at saying "I love you" while joohwan feels awkward saying it since he never used it before... Does make me wonder if sihyun ever admitted his feelings to himself and said that word before to be so nonchalant at expressing that even tho joohwan is his first in everything.... they're both Korean I doubt they'd be used saying it to family either.
