If I'm following the story correctly, the reason the guard Taeso has omega traits (ie a womb) is because when he was a baby he was used as a drug mule and had drugs smuggled inside his body at the prison. The drugs mimic omega estrus and had a lifelong affect on his body. Yes he is still a pregnant 'alpha' but its not crazy dick that made him change in to an omega, the pheromones but stimulated what was already there.
The drugs are probably going to be part of a larger plot line I assume since they're mentioned in a few different places
I know a lot of people like the side couple but they're kind of lame. They are the most normal white bread bland omegaverse couple that exists.
An alpha becomes an omega because he fucked too much, he gets pregnant, there's a ton of misunderstandings, they aren't honest with each other, they refuse to communicate through words and just bang all the time, the Omega runs off with the baby, there's a chasing arc, then they're happily ever after and they have two babies one that looks exactly like the alpha and one that looks exactly like the Omega. Blah blah blah, we've all read this a million times before.
