    The german in me is itching to clarify that it's unlikely specifically Alzheimers or whatever type of dementia.

    So. If he started showing symtpomes of Alzheimers this early on, this intensely too, he'd be stuttering and struggling to even form sentences and have the short term memory of a goldfish. Like he'd start talking about his love story, which in most cases I've seen he wouldn't be able to be this coherent and eloquent, and loose the thread (and in a bit to conceal his own confusion go off on a vaguely related tangent, imagine that happened in the next chapter lmao).

    I haven't seen patients with Alzheimers hold out for more than 10 years, typically it's more like 5. Depending on the person they're almost guaranteed to be a living veggie by year 5. They can barely express themselves and are bedbound bc of other age related illnesses, but also a bunch of them overextert their bodies due to a swiss cheese looking brain that's telling them they got to go back home to their babies, so they keep wandering until they fall.
    And there are methods to prolonging the progress of Alzheimers, but even that wouldn't be able to gift you +20 years of staying this clear - unless the manhwa author makes smth up.

    It could be a type of temporary dementia he is suffering from, but I would discount Alzheimers. If he didn't suffer a concussion then (I think) it is most likely to be a seizure that cut off blood flow to the brain for too long, making him black out like he did.
    May be epilepsy, or narcolepsy if he fell bad and hit his head, but at this point I am guessing myself. Could also be a much more complicated illness that affects blood oxygen or what have you, which I wouldn't discount. It had to be some serious shit for him to tuck his tail in and leave.
    I work with dementia patients, so yea.

    Probably some kind of space occupying lesion some one in the comments mentioned tumour...could be possible though amnesia is not the first sign of it...but well who knows...probably was in the hipocampal region....
    I do agree that it's unlikely to be alzheimer's....he is too coherent for it.
    Any other form of dementia would also be wierd for the same reason....
    It could be seizures....but wouldn't require him to go back to America for treatment....
    He does have low blood sugar issues...I think that would be adrenal or pancreatic origin?
    Low blood sugar can also cause altered mental status..like confusion.... (=・ω・=) I like when they try to make things as medically accurate as possible

    Yea I was thinking something more serious got to be going on, and good call with the blood sugar that could be part of it too.
    Gotta be patient for the next chapter and uncover this medical mystery fr lmao
