I think they maybe aren’t trying to make them look realistic and it’s intentionally over-dramatized. Just like how yaoi rarely follows human biology when it comes to things like how a prostate works, the location of sexual organs, refractory periods, anatomy, etc. Usually this is an indicator the author is prioritizing fantasy over realism. The author might not even be attracted to men at all, technically the yaoi genre was started by gay women!
