Ciel's twin killed Agni because remember when Soma punched Ciel? Because Soma has mistaken him for his twin when it's his twin that killed Agni.
But Ciels twin had someone with him when he attacked Agni, who said something like 'Allow me' - before taking over in fighting Soma and Agni (if I remember correctly…) so it could have been Tanaka perhaps, if the real head of the family ordered him to do it then he would have to have complied :(

It couldn't be Elizabeth since the person in disguise was a man and taller. There is a possibility that it could be Bravart though.
I can't see it being Elizabeth at all (unless Sensei wants to pull a GOT move and go totally left field with the story structure) (⊙…⊙)
The obvious scenario would be Bravat I think, and that Real!Ciel is the Blue Star person which he serves (and Real!Ciel only appeared now because he's spent a long time getting to this point through various means while he knew Fake!Ciel was out there living the life he was supposed to be living, thus building up more and more resentment toward him and his.)
