Each kingdom has like a boss (Grand Duke) who obeys the ruler (player) if they're a good king, but has the ability to Kill the player if he makes too many wrong choices. If your player character dies, you have to restart with a brand new kingdom and a new player will become the king to that one.
Dude is obsessed with game because the characters responses feel super realistic and kinda develops a relationship with them. He does something controversial (in game, not like morally wrong) and his subjects reacted badly. After a conversation with his Grand Duke that goes so poorly the Grand Duke tells him he hates him, Dude drops the game in depression.
Couple of months pass by people get obviously isekaid into the game as kings of the nations they built.
Dude is now having to deal with both the controversial action as well as abandoning the kingdom (the characters remember that he dropped the game). The biggest hurdle is his Grand Duke who is obviously PISSED.
This is SO worth it, my summary doesn't do it justice but trust me, it's worth a read, the characters are interesting, the plot flows pretty well and the world is pretty decently built and everyone is SO PRETTYヾ(❀╹◡╹)ノ~
