Well he is a boxer too lol
I did not know it was was a thing, I just looked it up and apparently it caused by blunt trauma from repeated impacts in combat sports, especially boxing.

Boxers, MMA fighers, or high contact sports that can cause damage and bleeding to the soft tissue of the ears can have it. If the blood isn't removed in time from the area and it becomes trapped & congested, that's why you see people draw blood from the area with a syringe, then it can lead to scarring or deformities like cauliflower ears
