As someone who has had micro tears from pooping and as someone who has had forcible anal intrusion, they are nowhere near the same level of pain. I can tell you for certain that it’s nowhere near the same pain nor the same healing. Pushing out and pushing in are not the same considering only one of those is something the body is made to do. That’s like saying pushing a baby back through a cervix is nothing because pushing a baby through a cervix is possible and done frequently across the globe. Also the blood was not just from the rim ripping, it’s from the colon being stretched and ripped and THAT is something you go to the ER and possibly go into surgery for. It’s a huge deal.

Anal ripping pain is one of the most excruciating pains cause it feels like the body is literally ripping in half and it’s connected to a vital organ and he’s just champing it. Whatever he’s gone through, it’s worse than death and worse than being ripped in half.
