i think it's just mainly for the fact that the stuff he can use are very limited in the age. if he was sent to another world with modern medicine too he'd have had no problem doing his jobs as a surgeon.

I love this. This makes everythibg more humanly, like sometime he forgets a procedure or his not prepared. Unlike other isekai making MC perfect.
