The "master" is the guide, the doctor (esper) is the one who killed the master. the body that the doctor is in right now, is the guide. Basically it's like he (doctor) possess the guide.

He is originally a guide too, i think (i dont understand that part much) and he's looking for the most ideal guide, right? And he found our mc, now the real reason is he's looking for a guide that is kind of similar to the guide he killed

In the end the sick doc was a victim of experimentation. Became obsessed with an esper so bad that he'd kill him, assume his body and spend an eternity to find another body to recreate his love. Damn crazy psycho.
