It was kinda obvious it was gonna happen. He literally likes getting his ass beat. That's why he does the matches that the residents bet on. I assume it won't be "resolved" until much later in the story. It looks like it was implied that he has some kind of illness which is either the result of the boxing or came before but either way will eventually kill him if he doesn't stop.
Stop while you're ahead.
the choking is intensely uncomfortable, I hope it is a focus of the story that gets resolved and eliminated as the story progresses
