Who gives a shit. It's fictional. This whole omegaverse thing started from fanfic then mangakas picked it up. The rules of this alternate universe morphs and changes depending on different authors who have different takes on it.
the 'rules' are bent here. in this artist's omegaverse ANYONE of any gender and second gender can get pregnant, unless theres something wrong with them, just like real life, where any women can get pregnant, unless there is something wrong. omegaverse are different in any story. every omegaverse universe is different and varies from author to author. this artist, anyone can get pregnant, alphas beta and omegas, but in another on, only omegas can get pregnant. the beginning chapter explanations are the same in every omegaverse story, just to give people who dont know, a VERY rough idea of what ABO is. i wouldn't take the explanations and apply it to every single omegaverse story, that would be stupid, because there are no rules to what people can or cant add to ABO stories. its just there to tell you what ABO stands for.
The beginning of this manga (the part that explains omegaverse) omits the part where they explain the pregnancy so it could be inferred that everyone can get pregnant.
