I'm pretty sure that the brother is either a foster brother or half brother because Kanae refused to call him onii chan. But it is not stated in this series.
I've seen several stories where relatives have to prepare for an omega's heat so that they don't attack them. Honestly, there are lots of messed up things about the omegaverse, so I don't necessarily think the risk of incest is the worst. Sad that it went that direction here.
