I think maybe the time lapse isn't very well conveyed. Heesoo had to listen to Hyesung cry and beg for probably hours considering he gave him meds 5 times before he resorted to masturbating which Heesoo had to listen to long enough that Hyesungs hands were so sore he couldn't do it himself. Then he tried to lend a hand but to no avail. I imagine that having to endure witnessing all that gave him new respect for the person who has to live through it and her felt guilty for what he's done. Contrary, Hyesung cares very little about the lies and more about running away from his life and finding a "cure" as he originally thought he was an alpha. He never wanted this Omega life.
